Liability Waiver  

 

For and in consideration of my entry to Sliding G Ranch, LLC (SGR) or use of any equipment,  enjoying any sport or dog training class or for any other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I intend to be legally bound and here by for myself, my heirs, assigns, executors and administrations waive and release any and all rights and claims for damages or injuries to me or my dog arising out of or in any way related to my participation in any event, class or rental or just general use of the property at Sliding G Ranch, LLC (SGR).  I also agree to hold harmless the contractors, agents, employees and representatives of Sliding G Ranch, LLC from same.  I understand that this is a dog facility, and I am entering at my own risk.

 

Publicity Release  

 

I hereby grant Sliding G Ranch, LLC (SGR) right to record, broadcast, photograph, film and videotape my participation at SGR and related interviews and to utilize such recordings, broadcasts, photographs, films or videotapes together with my name, image, likeness, voice and biographical information, for such publicity promotional or commercial purposes as they may desire.  I understand Sliding G Ranch, LLC (SGR) may assign the right to use the recordings, broadcasts, images, photographs, film and videotape of my participation at SGR together with my name, image, likeness, voice and biographical information to such person or other entity (including but not limited to a television broadcaster) as they may desire without further consent or knowledge and I expressly agree to such assignments. Finally, I recognize and fully understand and acknowledge the information above. By signing this form, I expressly declare that I am of legal age and that I am aware of the full contents of this liability waiver/ publicity release, and I am legally accountable to any effects of this
